Dear Parent / Carer Re: Year R Health Needs Assessment

The School Nursing Service routinely visits schools as part of the Healthy Child Programme and to support children and young people with a range of physical, developmental and emotional health needs. The Healthy Child Programme recommends a number of health and development reviews with a core purpose to:

• assess the child’s, young person’s and family’s strengths, needs and risks,

• give children, young people and parents/carers the opportunity to discuss their concerns and aspirations.

• assess physical health (inc. any medical needs), growth and development and immunisation status.

• assess any mental or emotional health issues.

• ensure that appropriate support is available to meet educational and health needs such as language and communication.

The School Nursing Service is inviting you to complete a Health Needs Assessment Questionnaire for your child. This is an opportunity to have your child’s health and development reviewed. We are now using an electronic method of communication which is a faster, more efficient, and safer way to deliver your child's health information.

Please take a few minutes to read the information provided. This will give you a clearer understanding of what it is we aim to deliver.

What do I need to do?

STEP 1: Using the link below please either register for an account if this is your first time using the parent portal or login if you have already created an account. If this is your first time registering, you will receive an email to confirm your account and set a password.

STEP 2: Once you login please complete the questionnaire about your child’s health. By completing the questionnaire, this will help the School Nurses to understand if your child needs any support with their health and development and may contact you for further information, advice and support.

If you would like to discuss your child’s health or development further, please contact the Health Hub on the above telephone number, the administrator will ask for your child’s name, date of birth and the school they attend so they can identify the School Nurse team in your area, who will return your call.
