Dear Parents/Carers

All children from Reception to Year 6 will be offered the flu vaccination this year. This will be administered during the week of 30th November. 

Please find attached the parent/carer letter containing the link for the flu E-consent form and full details regarding the immunisation programme.  Please complete and return the form electronically and submit it by 27th November 2020 (please note if you have more than one child in school you will need to complete one per child).  The form will then be sent directly to the immunisation team. Should you have any difficulty completing the form please contact the immunisation team (details on the attached form).
